REQUESTING AN OFFICIAL
TRANSCRIPT
Please allow five business days for transcript processing. All transcript requests must be received in writing with a legal signature.
You may use our transcript request form or you may write a letter.

If you write a letter, please provide the following information:- 1.) Your full name and any former names you may have been enrolled under
- 2.) The degree or program you were enrolled in
- 3.) The approximate last year you attended
- 4.) The last four digits of your social security number
- 5.) Your home address
- 6.) The address that the transcript should be mailed to (if different from home)
- 7.) Your phone number or e-mail (we many need to contact you about your request)
- 8.) If you need multiple copies for graduate school applications, please request them "in separate sealed envelopes."
Students who have an outstanding balance on their student account will not be permitted to have an official transcript without approval from the Business Office. If you have an urgent need, please fax your form or letter and mark it "RUSH." Request forms or letters can also be scanned and e-mailed as long as there is a signature. THERE IS NO CHARGE FOR OFFICIAL TRANSCRIPTS. Donations are welcome and will be directed to the Campus Needs Fund, which is a source of emergency funding for students and members of the MTSO community.
